Once you've signed up to become a member you can access your patient records or schedule your next primary care appoinment online!
To access your medical records or schedule an appointment,
or, If you just need to schedule an appointment you can use the appointment scheduler below.
*Please remember that in order to be seen you must have an active membership. If you are not currently a member and you submit a request for an appointment we will send you information on the membership requirements.
You can also head over to our "Become A Member" tab and get started right away!
* After you've selected your appointment time and entered all of your information, only click the "Finish" button once and wait for the confirmation screen. Confirmation screen may take up to 2 minutes to appear.
